I'm not sure if I have seen this problem before, but I just noticed a new bug (or feature?) when signing via Command Line from the 0.9.4 SDK.
Here's what I do:
[...] 77 7b 3c 17 ad f1 3c a7 a3 8a ad cb db 41 b7 30 2f 53 4e 6e 1c c0 93 b1 df ba fb updating: META-INF/MANIFEST.MF adding: META-INF/RDK.SF adding: META-INF/RDK.EC signing: air/bgmusic.mp3 signing: air/biddy2.png signing: air/blackberry-tablet-icon.png signing: air/blackberry-tablet.xml [...] bar signed.
I left out the less interesing parts, but all files (XMLs, SWF, etc.) are included.
No error message at all.
BUT: The META-INF folder contains only these three files:

@biggerCC, you're overlooking the "double-sign" step. See the HOW TO for more. It's the command that ends with "author":
blackberry-signer -keystore MyKeystore.p12 -storepass MYPASS MYAPP.bar author
