Although my apps are just perfect sometimes users request a refund.
If the users have purchased an app via carrier billing, I advise them to contact their carrier.
If the users have purchased an app with credit card or PayPal account, I advise them to send the purchase order to
email@example.com and to request a refund.
Is this the correct approach to handle refunds?

You're 100% right on both.
For carrier billed purchases,the carrier handles refund support. By the way, did you know we recently launched carrier billing on Vodafone UK and Portugual and A1 Telekom in Austria?
For cc/PayPal billed purchases, firstname.lastname@example.org is the way to go.
