My App was denied and I needed to change my app name (app ID remained the same). Since the app name was part of the LE registration form, do I need to submit a new form?

I'm curious: why did you need to change your application's name?

Yes if your app failed and you would like to re-submit you just have to re-submit using the form as you the first time. The name app is fine and shouldn't cause any issues.

I had to change my product name for trademark reasons.
