how can i reset factory settings

i tried to load blackberry on to my desktop and it has changed my phone settings - how can i get it back to what it was??

ive lost all my numbers and everything and there is no longer a keylock thing either

ive lost most of my things - phone numbers and emails, the keylock button -

so if anyone knows how to restore settings? phone numbers? that would be great
