Hello I would like a refund on the above charge. The app was not like the description.

Your inquiry needs to be sent as follows. Go to this URL:

and fill out the form. Use, in the "Please select a component:" field, the AppWorld Billing/Payment selection.

I have attempted to use the above form and it will not accept my PIN.

Is there a contact email address that I can directly email to inquire about a refund?

There are some problems on those forms rejecting valid PINs. To fix that, I have sent you a Private Message on this site with instructions as to how to deal with that. After that is resolved, you will be able to enter your request on those forms.
