I have never received a notification for upgrading my device. Are you sure it isn't some other applications such as an email, facebook, blackberry messenger, etc. that is the source of the notification? Maybe this is some new thing with OS 4.6 (I have 4.5). Perhaps somebody else can confirm this.
What does the icon look like anyway?

I am having the exact same problem. my current version is 4.6.0.167, it says "upgrade to 4.6.0.297", when I select that version I get the same error, "upgrade request failed. try again later or upgrade using blackberry desktop manager on your computer.
When I select "details" and then "send report", it seems to indicate this file does not exist on the server?

I might suggest that OTA updates are never a joy. 1) Long download process, subject to interruption on the carrier network. 2) No automated backup pre-update...some have reported losing all of their data with an OTA update.
Tethered updates are, IMO, better as they alleviate both of those issues. I suggest the following steps (insert plenty of reboots of your PC...not just restarts, but full power down reboots). Also, it is advised that you be logged into the PC on an account with full admin rights. Further, under Vista, use the "Run As Administrator" option for everything. If you have never installed the DTM software, then you can skip steps 1 and 2.
1) Remove your device OS update package from your PC (add/remove programs)
2) Cleanly uninstall the RIM DTM software:
3) Get a fresh download of the RIM DTM software:
4) Download (to your PC) a fresh copy of your device OS package from your carrier:
5) Install DTM to your PC
6) Install (also to your PC), the device OS package
7) Tether your BB to your PC and launch DTM...the update process should be automatic. Be sure you accept the option to perform a full backup...if it does not offer you the option, stop and manually execute a backup. Then re-start the update process.
Hopefully that will get things going again.

Just to let you know you can simplify this as you dont actually need the Desktop manager software to do an upgrade.
Just download the OS software from your carrier (as per Step 4), then run:
C:/Program Files/Common FIles/Research In Motion/Apploader.Loader.exe
And follow the prompts.
