Desktop Manager 5.0 does not prompt for new OS

I downloaded the latest OS for my BlackBerry Curve 8330m and deleted the VENDOR.XML file. I also backed up all my data and apps. According to the instructions on CrackBerry, when I plug my Berry in and start DM, it is supposed to prompt me to install the new OS, or if it doesnt' to click on App Loader and it will prompt me there.
It doesn't prompt me either time. How do I actually install the new OS?

was the upgrade from your actual carrier, if so you dont need to delete the vender.xml file. Deleting the the vender file is only needed when you are installing another carriers version.
and you have to delete the vender file after you install the device software to windows.

The upgrade was from another carrier, either Alltell or US Cellular.
I'll check that Options thing again.

Here's a screencap of the upgrade options page. Should there be two 4.5 OSes listed?

1 is system and the other is core... so yes anything greyed out, you can not change. but action says none beside each one, so they wont be touched, so when you get to the last screen, choose advanced... this will give you the iotion to perform backup and restore, but also perform a complete erase and reinstall, which will force the upgrade.
also double check add/remove programs in control panel
make sure you see your desktop software and the NEW BB software and no other BB software
