The service book site asked for a user name and password.  I don't recall either of those so I asked for it to be emailed to me and when the email came through it stated that user name and password don't exist.

 

Looks like I may need to take it into a store.

On your device--the BlackBerry--open your personal email setup icon, and there you can resend the Service Books.
