Each time I turn on my new phone, before the password promt, I get a start up screen of 'My services' witha SIm card icon requesting me to select one of: 'Make a payment', 'My account', Horoscopes', 'Music', ......'Chat'. How do I stop this happening each time I switch on the phone? Thanks

What you are seeing is not controlled by the BlackBerry, but is a message from your mobile provider.
The BlackBerry can't control that.
Are you on a pay as you go plan, or a monthly contract?
Tmobile USA, or Tmoibile UK, or DE?

The phone is only 3 days old. The first time the phone was switched on, it did not happen (as a video of how to use the phone started). I set up a passcode and ever since this message has appeared at start up. The only time it never happened was when I started the phone in safe mode. Is there a way of disabling this menu appearing at start up?

Ok, I've never seen this before. I don't recognize it as being something controlled by the BlackBerry, but it really does appear to be a message from Tmobile, or one of their apps like "MyTmobile".

Hi there...to add, given that it's only 3 days old, I'd recommend just taking it back and having your provider sort this out.
