I have the same problem and here is my theory. Do you have a password on the phone when it is locked? If so then this could lead to a security issue with in the handset itself. Once the phone is locked with the password then the media card is locked too along with the address book. Hence not allowing the phone to access the media card with the custom ringtones on it or the address book with the phones numbers or picture attached to that number. That is why it when the phone rings it just shows the number and uses the default ringtone. If you take the password off and lock the phone manually then the custom ringtone works fine. The draw back to this is that sometimes I am in a rush and forget to lock the phone. Also the information in the phone is not secure.
Some how this would need to be fixed, but I think it might create a hole within the security feature of the product and possibly leave all your information vulnerable to someone that finds it. Hopefully someone has a fix or suggestions. I hope this helps.
