After adding a Gmail account to BIS (OS 5.0.484) the Address Book folder is always called Gmail. This doesn't really cause operational problems, but after the sync (and even the service books) are removed, the name remains.
This makes for some confusion when configuring ActiveSync via NotifySync since the folder to sync contacts with is always called Gmail, not Contacts.

You can clear that address book completely should you desire:
Do a backup first using Desktop Manager.
1. Open Desktop Manager on your PC.
2. Open up Backup/Restore > Advanced.
3. Highlight on the right side your Address Book database and click the button to below CLEAR the address book from the device.
Now sync your address to the BlackBerry again.

I will give that a try. Does that actually remove the address book (and something forces it to be recreated)? Or does that simply remove its contents?

I tried this, and while it indeed removed all contacts I had, the Contacts folder is still Gmail.

Have you considered a complete wipe of the device?
Options > Security > General Settings > Menu > Wipe Handheld?

Definitely have considered it, was just hoping there was a way to resolve without doing that. I may do it today, if I have time.
