The article KB04099 show all the specific errors for each component, for the Dispatcher the error 5408 is "Unable to load Lightweight Directory Access Protocol (LDAP) API library", check in the BlackBerry Server Configuration that the LDAP settings are ok, under Administration Service - LDAP.
I was never able to trace what occurred but given that the system was a test box we simply wiped it and reinstalled everything before allowing people access to the BES.
