Install Issue - BES 5.0 "SQL Query Failed, See logs for information"

We had to leave the setup halfway through for a couple of weeks due to port access issue with RIM (tcp/3101). Now the port is opened and we continued the installation. The installation exits abruptly with the following msg. "SQL Query Failed, See logs for information"Attaching the log & Screenshot here. The SQL server seem to be running on the server, but the installations fails.

Any idea on how to resolve this.

Is the MDS-IS install on the BES , because it seems : "Failed to insert a new record for machine name into MDSIS database"

I think when MDS-IS is installed , it's not advise to upgrade to BES 5.0 .
