I'm getting this error on 2 users in ALRT log as well as a similar entry in Event Log:

I've recently added 3 users, split all the users into 2 Organizational Units because they are using different domains and I can easily assign e-mail address policies like that. There are 16 out of 17 users in the first OU that work, and 3 out of 4 users in the second OU that work.
Above error happens on two users on two separate OUs/domains, while there are 19 users that work perfectly fine (split across the same two OUs/domains).
I have no clue what's going on here, IEMSTest fails with OpenMsgStore failed, but most of the users work fine. I have set all the permissions on BESAdmin over from the beginning, reinstalled BES, still nothing helps.
Configuration:
1st Server:
Windows 2008 64-bit
Domain Controller
DNS Server (authoritive for one of the domains)
Exchange 2010 (authoritive for both domains)
2nd Server:
Windows 2008 64-bit
BlackBerry Enterprise Server for Microsoft Exchange 5.0.1 MR
Windows Firewall is turned off on both servers, they are on the same subnet, each with static public IP addresses, we've even gone as far as disabling the hardware firewall.
There's a third Linux server that handles DNS for the second domain, doubt that would be relevant though.
I'm also getting a bunch of
CDOCalendar::ProcessWindowsTimezoneInfo- No Index value present
errors for each and every user in the ALRT log, this has been happening the past two days (above issue started today). I've tried the info in the KB about Mauritius Standard Time, but we're on Central Time (US & Canada), and apparently this issue is not supposed to occur on BES 5.0.1.
Lastly, another error that's been on this server since day one, but never really caused an issue:

I've tried the steps in both, however, the problem persists.
Earlier I had the problem with 3 users, now it's down to 2. I fixed the other user by deleting it from BES and adding again, however, that didn't work for the other 2.
Strange how there are no problems with most of the users, only 2 giving these errors.

Repeats many times, though I've created a new service account, followed step-by-step the KB to assign permissions, etc, all the usual stuff. Above only happens for one user (but not the 2 users giving problems, another user).
I had a user report that he can't send mail (the user with the MAPI_E_FAILONEPROVIDER errors). Most of the phones are not actively being used, so it's hard to tell whether this is a single case.
I'm still hoping some generous genius can help me out :-). I'm at a complete lost here.

I had the same problem. Followed all the docs on bes5/Exchange 2010. Looks like RIM's docs are incomplete. Had to run the following command on the Exchange 2010 server to resolve the issue. Set-ThrottlingPolicy -RCAMaxConcurrency $null -identity BESPolicy See here.

I think this is gonna work, I did it a bit differently though, as I didn't have a BESPolicy to begin with. So I created one with RCAMaxConcurrency $null and applied it to the BESAdmin mailbox.
Will let you know how it turns out.

In a Microsoft Exchange 2010 environment, there will be a need to turn off Client Throttling to allow our MAPI connections to the Message Store. By default this is enabled. The BlackBerry Documentation is incorrect when explaining the command. The command is missing a "$."
After the command from the BlackBerry Documentation is run, and when the BlackBerry Enterprise Server is started, some users may start but randomly some may fail.
The MAGT logs may show the following:

I am having the same issue only i have already set my throttling policy correctly. I have been on and off the phone with RIM for hours today and was hoping someone might have some suggestions.

Hi,
I have a costumer with the same problem.
Even tried to install a new server.
The throttling policy is set, but as soon i create / activate the 21st user it stalls with the same error.
Did you find a solution to this ?
