According to http://support.microsoft.com/kb/905803 any folders which have over 5000 items in can slow down exchange. It sounds to me like lots of smaller items have a larger impact on performance than a few big items.
Does anyone know how we can clear the BlackBerryRefIDSearchFolder1_7 folder??

Try to copy 5000 items across the network that equal 50 meg compared to 5 items that equal 50 meg.
The 5000 will take longer, since 5000 connecitons need to be made.
Excahnge will do similar and access 5000 items.
Teach user to have less than 5000 items in any folder seems to fix it here.
This also speeds up Outlook as well since after 5000 items it will not cache the view of the affected folder.

BlackBerryRefIDSearchFolder1_7 is a hidden folder.
Any other ideas??

You can unhide that folder, been a while since I have used MFCMAPI.

But is it safe to clean up that folder? I guess it is there for a purpose but I haven't been able to find anything in RIM documentation.

No not safe to clean:
We have used MFCMAPI to delete a mail item from the BlackBerryRefIDSearchFolder1_7 folder, although the item is then deleted from exchange as well.

If you can, remove user from BES and when asked to remove extra data choose yes and it will remove the folder.
Then after 30 min (unless you can prove it has been deleted) then add user back to BES and see if things are better.
