My Carrier: T-Mobile

I have a 8100

Please help,

I have a peral 8100 I up graded to the iPhone, my corporate took my BB off the server but I still have to put my password in to unlock my phone. I called T-Mobile and we did a reset on the phone which took about an hour but that setting is still there. I want to put this phone back to factory settings so I can give to my daughter... The phone is still in use... What do I need to do to make it like it came out of the box?
