lorriem1222 is right! PUK is the property of your SIM card. If you enter wrong SIM PIN no. 3 times then you'll be asked to enter PUK code and if you enter wrong PUK code 10 times then your SIM will be dead forever!!!
Usually PUK is written in the intial SIM card package by your carrier. They also have a log for PUK codes but often carrier don't let people know about PUK code. Rather they induce subscribers to get a new SIM card with same mobile number.
