Duplicate Entries in Contacts

I had problems with this once before but for the life of me I can not find my original post so I will start over.  Due to problems with my Storm Verizon gave me another phone yesterday.  My problem now again is no matter if I create a new contact in Outlook or on the Storm I get a duplicate entry in the Storrm.  It puts one in by first name (which is what I want) and one by the last name.  Even if I create if from the Blackberry before a sync is even done I have the two entries.  I thought I had to go into the Blackberry settings someplace and tell it that all new entries by first name.  But I can not find that option anyplace.  Any help would be appreiated.

Forums and old guys gon't go together.  This is the post but this solution while worked the last time does not work this time.  I still get the two entries

I am 41 years young.
