I just instaleld a BlackBerry Enterprise Server 5.0.3.12 on a Windows 2003 Server with Exchange Server 2003. Now I'm having this problem with the BlackBerry Administration Service web interface, it don't accept any login/password I input.
I already tried using it in all formats I could imagine, Username, DOMAIN\Username, username@domain.fqdn, already reviewed all the LDAP settings, I'm using the Administrator login just to make sure it wouldn't fail, but still nothing.
The log don't give me any errors, and failing to login don't add any line to the BBAS-NCC log either. I've tried every solution I could find online, even thought I wasn't getting the same error message as they were. Updated my Java and stuff, nothing helped.

Since you tried everything online and it still wont open even after a reinstall of BAS. you might have a serious problem and will require a support call.

It looks like your logging detail is set to Informational. This won't capture failed login attempts.
Increase to TRACE as per http://www.blackberry.com/btsc/kb00765
