Somehow I have managed to delete the compose SMS button in the main menu - how do I get it back?

Try going into the menu, pressing the menu button and choosing Show All.

If it appears then, select it, press the menu button again and uncheck Hide.

By menu button, I mean the physical button below the screen that has the blackberry symbol displayed on it, the second from the left.

Actually, this just happened to me - my SMS Text button just up and disappeared.  It's not hidden and it wasn't moved to any other folder.  I would like to get it back.  Or am I being told by the might Text God that I shouldn't be texting?  lol
