Thank you for your inquiry, this would be caused by a paired Bluetooth device. Go into Manage Connections > Networks and Connections> Bluetooth > Here you can see your Paired Devices, delete any you are not using.

Unfortunately that didn't work. I deleted all blue tooth devices and only kept the one I use the most. But the message "Connection to audio device failed" still keeps popping up. Any other suggestions?

What do you have paired with the BlackBerry smartphone? Try testing each Bluetooth connection you have paired with the BlackBerry smartphone to determine which one is causing the issue.
Let me know how you make out.

I have deleted all the blue tooth connections, then pulled the battery and put it back in. After a fresh start up, the message still appears - "Connection to audio device failed". ....This is driving me crazy.

I have a bold 9700 and recently paired it with my 2011 Nissan Rogue and started getting the "Connection to Audio Device Failed" regularly when not paired with the Nissan Rogue's bluetooth. There is also a Plantronics Voyager Pro + paired. The only solution I have found is to shut off the all bluetooth conections. I would like to know how to keep the bluetooth functioning but just not have the error message constantly interrupting use of other applications.
