Hah I found the problem. I had to install the latest JAVA runtime and then it worked. Before that it didn't work for me in IE8 nor IE9.
Kind of odd maybe for a site to require the latest JAVA runtime to submit a form?

I'm having this same problem now, completely blocking me from adding an application, with no messaging to give me any idea what's wrong. Very poor UX design, IMO.
Is this seriously a problem with latest-Java-runtime installation? Has anyone else having this problem found this to fix this problem? Seems very odd that that would be the problem here...

After I was told it only worked on IE I tried it on IE8 on my wife's computer. Didn't work. Then I installed IE9. Didn't work. Then I noticed a pending update for JAVA and installed it. After that I thought "What the heck, let's try it one more time" and... it went through.

No kidding. I just updated my Java VM installation (from v6 update 23, to v6 update 24), returned to the "add product" screens in BlackBerry App World, and the product-icon-upload form seems to be working just fine now.
