I removed the Appworld 4.0.0.63 though Apllications Management and the uncaught exception :java.lang.error went away for 3 days.
I redownladed AppWorld 4.0.0.5 and the error has returned. I also still have no AppWorld Icon even after battery pull & reboot.
I have a Torch 9800 with 6.0.0.2921

Open the BlackBerry Browser on the BlackBerry smartphone.
Browse to http://mobileapps.blackberry.com/devicesoftware/entry.do?code=appworld3
After download, please restart your BlackBerry smartphone when prompted.
Verify that BlackBerry App World is located on the home screen.
BlackBerry 6 smartphones with an upgrade path to BlackBerry 6.0.0.706 OS or higher should upgrade to the latest version of BlackBerry Device software available from the wireless service provider in order to resolved multiple issue with BlackBerry Balance.
On BlackBerry 6 OS versions 6.0.0.706 and higher and the App World Icon is not visible, please download and install the latest version of BlackBerry App World.

@JSanders : I am on 6.0.0.695; ignoring the AppWorld upgrade prompt (for 4.0.0.63) after the initial issues and reverting back to 4.0.0.55. I guess that's the safest option for now

I've tried all the solutions. The application management says I have the app world 4.0.0.65 installed but I can't find the icon.
Device OS version number: 6.0 bundle 1306 (v6.0.0.359, platform 6.5.0.93)
Device type: bold 9780
Rebooted: Yes
