How to pass event to check box

Some one please help me passing the checkbox data to another screen which will display summary something like,

 

Checkbox 1 not selected.

checkbox 2 selected.

checkbox 3 selected.

I am able to create the dialog box. But it would be helpfull that how to load the fields inside dialog boxes so that mine field data I can populate on next screen
