Unable to sync address book

I've always had some issues with contact sync, but despite errors it seemed to keep working.  Now when I open my BB software the address book sync box is grayed out and it says it's "not installed"

 

Most (but not all) contacts remain on my phone and are available to email or SMS, but when I open the Contacts folder itself it only shows one contact and it repeats it several times.
