Users are reporting in the reviews that my app doesn't work. One left a review stating how it should work according to what they think it should do. They obviously didn't read the instructions because the app DOES work. What should I do? This hurts sales.
Also, if a user ever wants to update their review, they can't. Why are reviews permanent?

You can deny reviews from the Vendor Portal, under Manage Applications. Maybe you should consider re-writing your app description to be more clear.

It would still be nice for users to be able update their reviews. I hope that is a feature in the next version.

I also wish there was a way to contact reviewers, even if they remeained anonymous. It's annoying not being able to tell them how to do something, if they don't know how.
