when I call blackberry.app.exit() in my app, I randomly got an error message "Previous instance still active (report to http://go/memoryleaks)" .  However, if I exit the app using the menu/Close (which is something in the menu by default, I never have any problem.

Whats the difference between blackberry.app.exit() and the default menu/close provided in the menu?

Are you able to provide a small sample HTML page that produces this issue? Also, can you please confirm the WebWorks SDK version you are using and the device model/OS you produced this on?
