For testing as in debugging?
You can also use the debug tokens that were sent out to install your app without signing it on the PlayBook

What if it was a signed bar file? could I send that to a friend who has a PB so they can test it out for me (hypothetically speaking, say my app was paid and I didnt want them to put in their credit card info)?

You can use Sandbox mode. Add that user BB id using vendor portal and then he/she will be able to buy application from App World without paying. But yes, that user need a paypal account.

What I was trying to get at is for quick testing on the real device. I guess I can use the debug thingy but it would be nice to sync the PB app just like on the BB and make sure everything works before submitting to AppWorld.

The only way I know of to install an application on the Playbook outside of App World would be to use the blackberry-deploy comman line utility. For that, your friend would have to install the SDK.
It would be nice if there was a way to install OTA updates from other websites. I have no idea how else you would roll out software to clients.
