No, it's free to sign up with AppWorld right now. It's free to submit an app. It's free to get code-signing keys.
It does require a CC which will have a $1 hold put on it.
Yes, you need to codesign your app in order to load it onto a real device.

Where can I find the extra registration form for individuals mentioned in this Topic?

Once you submit the app world vendor application, they will send the form in a seperate followup email if you will be operating as a 'sole proprieter'.
