I don't know the specific changelog of .4687 over 10.1.0.273, but I (me, personally) wouldn't be afraid to upgrade, especially if there are issues with .273 on your device. It is a leaked OS, do it at your own liabilty, but it is possible to always downgrade the OS again should you wish to. Just make sure you do a backup first to Link using the .273 OS should you wish to return to that OS.

Are you aware of any carriers issuing 10.1.0.4687? If one or more carriers have issued 2354 it might be safer to upgrade to that version first?

If the Mac is running Bootcamp or Parallels to run Windows, yes.
