I have a client on Blackberry Enterprise Server Express (running with Exchange 2007), with a blackberry 9000. If he creates a new contact in Outlook 2003, it does not show on the handheld. If he edits a current contact in Outlook, the changes do get pushed. If he creates a new contct or edits a contact on the handheld, the changes show in Outlook.
I have deleted and resent the Desktop [CICAL], Desktop [SYNC] and Desktop [CMIME] service books. I have restarted the Blackberry Administrator Synchronizaation service and resent the books again, we have even wiped and re-activated the phone, the issue still persists. I have verified Wireless Sync is on for his Desktop contact list, and that he is indeed adding the contacts to the Exchange Mailbox and not a Personal Folder.
Is there anything else I can try? We have already spent a lot of time and money on this, and would like to get a resolution quickly.

Start with this.
In outlook Make a new folder for contacts called backup
move all contacts to the folder and wait for all to be removed from device
then pull battery on device and put back in
After device boots move contacts from backup to the contacts folder and wait for them all to appear
Make a new contact in outlook and see if it gets to the device.
if above fails try this next.
