Can you have a single standby node service multiple primary nodes?
Similar to an Active-Active-Passive clutser design.
The other two choices I am considering is to have a single sql clutser, two physical BES servers and two VM standby servers.

The BES instance has a 1 to 1 relationship in terms of primary and standby servers therefore it is not possible to build a active-active-passive cluster as you suggest. The BES maintains health scores of both the primary and standby servers which it uses to determine if it should failover to the standby node which also prevents the 3 node cluster setup.
I would be weary on using a SQL cluster because as far as im aware SQL clustering is not supported. To get high availability for your SQL databaase you should use the database mirroring feature in SQL 2005. BES 5 has inbuilt support for this feature.
