"occasionally"? That is odd.
Where are your ringtones stored, on the device memory or media card? Do you notice they don't work when you are connected to the USB cable on the PC?
What OS are you on? Look at Options > About, third line down beginning with a "v.4.xxx"

When I first updated, they did not work. The only way I found to fix it was to R&R the battery. Then they worked for about one day before I had to R&R again. This time they worked for a week, plus. I haven't been able to find anything that seems to trigger them to stop working. Maybe it's a memory issue, but I seem to have plenty available.
They are stored on my media card and I'm running 220.127.116.11. I haven't noticed whether they work when connected to the PC.

did you ever find a solution for this problem? my bb stops playing all ring tones everytime i use the media player, listen to a song and press stop or pause. if i listen to a song all the way through and dont have to stop or pause anything, all ringtones will continue to work.
the fix when this happens is battery pull, and iv tried a wipe. any other permanent fix?

Mine too and I have newer software. Every time I change ringtones, from the collection provided, all alarms stop working. I have to remove/replace the battery. This seems like a software malfunction to me. It potentially makes the phone useless and at best unreliable.
