Verizon deleted visual voicemail and cant reinstall - suggestions??

Verizon had a glitch with voicemail and asked to delete then reinstall visual voicemail for the storm 9350.  only snag was that they didn't have accurate information to reinstall it.  subsequent "suggestions" from verizon have also failed. 


What IS the tested and proven procecdure to reinstall visual voicemail after the app has been deleted from the device?
