How to register the Blackberry push service?

Hi, we are developing blackberry application to support push services. So, we already tried the evaluation version that is available only 4 months. By now, we would like to register the paid version by this link.

But we have no idea to fill in all required fields. Such as ...

- Over-the-air Download Link (for example www.yourcompany.com/yourapplication)

- Base Notify URL (Domain where response notifications are sent. Example: https://www.foo.com)

- Push Tier Selected

- Annual Fee

Do I go to the wrong page? Or something else?

Really need help.

Thanks in advance,

Same here, the bb push service form is very ambiguous. Even the email we received after the eval didn't provide much info back to us. Keep forgetting the base notify URL on mine
