Several possibilities, here's a few;
- You've recently updated the OS and it's become corrupted
- The Service Provider ahs removed it in their version of the OS
You are using BES and this has been disabled by your sys admin (note sure whether this is a capability of BES or not to be honest) [Edit] Just looked at the BES10 policies and suggestion 3 wasn't possible.

The trick when you had the camera was to move the green square to what you wanted to focus on to get great pictures.
I suggest trying all the usual rebooting options to try and get it back.
Try holding down the power button for 10 seconds and/or holding down the volume up/down buttons for 20s.
