I am getting inquiries with regards to users being charged over free upgrades. Is it possible for RIM to look into this?

I will reach out to you directly regarding this problem. We're investigating this issue as many Vendors appear to be affected since the PlayBook 2.1 upgrade.
If possible we'll require some more information from an affected user.
I will try to update this thread once I have more information as well.

Is this problem fix? I have made an update and the store is also asking me for the full price (for the free update). Now, I did purchase the game with a sandbox account, so it might be the reason (I thought) but now, reading that real customers are also affected is a bit scary. So, is this problem solved now?

In-app purchases and being charged for Free upgrades appear at this time to be tied to the newest version of PlayBook OS that went out.
Both have been identified and I believe a resolution is just around the corner for both problems.
Not a good user experience, agreed but we're working on getting the fix out.
