I'm pretty sure that Guigui77360 is correct in the comment "Auto-run applications must be digitally signed by RIM to run on BlackBerry smartphones."
Try signing your app with the 3 code signing keys, because your app will not run on startup if it's not signed with the RIM Runtime key. The code signing checks are skipped on the simulator but are enforced on a real device. You can enable the code signing checks in the simulator by selecting "Enable device security" in the JDE's "Preferences -> Simulator" screen.
You may have to make a bogus method call to an API that requires this code signing in order to actually get your app signed. For example, call net.rim.device.api.system.CodeModuleManager.isMidl

I tried my code with the box "Enable device security" checked on the simulator and it didn't work !
So I will wait for my code signing keys and see ...
