Does the application appear in the module list if you go to Options, Advanced Options, Applications and choose Modules from the menu? Is this a BlackBerry CLDC Application or MIDlet?
What debug output do you see from the BlackBerry Smartphone simulator when the application is loaded (if you load it from the simulator's File menu)?

Run the clean.bat file located in the simulator directory to reset the BlackBerry Smartphone simulator back to its default state.
Is this happening to any application in that particular simulator or just the one?
