Creating a basic form with a sheet

I am trying to create a basic form that fits the style of BB10.


I am wanting the form to look like the form on this page




How do I get buttons in the titlebar? What layout should I use for a basic for like the one on the page.


Thanks for any help or suggestions you might have.

I initially tried the titlebar, but I only got the splashscreen when the app ran. Must have messed up the QML somehow.
