Which video card do you have? The QML previewer uses specific extensions to OpenGL which are not supported by a decent number of onboard chipsets. The particular extensions seem to change slightly from release to release.
If you had it working very recently, and it just stopped, you should be able to force it to redetect your capabilities by going into the preferences (Window -> Preferences…) and disabling the QML preview (BlackBerry -> QML Editor) and restarting Momentics (File -> Restart), re-enabling preview, then restarting again.
It is a royal pain in the rear, and I wish there was a "redetect now" thing hidden someplace, but this has worked for me a few times now.

On Mac (which is not your case) this is caused by missing 32bit JVM.
I suggest to check $QNX_WORKSPACE/.metadata/.log for more info.

I have 2 versions of Java on my mac 10.8.2, 1.6 and 1.7, even after I changed the JAVA_HOME to point to java 1.6 which supports 32-bit, I still run into the qml disabled because missing 32bit JVM.
Would you know if it's possible to modified the momentics ide startup script somewhere to have it point to java 1.6 instead? Or is there any other solution out there?
Thanks a lot

Windows->Preferences->Blackberry->QML Editor, then uncheck : Disable QML preview, finally, restarting QDE.
It will check again QML Preview capabilities.
Work for me, great !!!
